March 2013 Update: Output from this web interface is a Postscript .ps file.
If needed, select and use one of these PS to PDF converters on the .ps file output:
  Online conversion: Google search ps2pdf
  Offline conversion for both Windows and Linux: install Ghostscript package on your computer.
  Install Postscript and PDF renderer from that site.

   Select built-in image, or upload a JPEG image file.
  2a: Select cover background:   ( Pictures ) Use image as logo, no scaling.

    Create Folding Insert for Slim CD Case
    Create Inside Insert for DVD Case
    Create Outside Cover for DVD Case

6b: Select options:
    Omit Title & Subtitle from Tray (more space for items)
    Omit Title & Subtitle from Cover
    Enable scaling down of items (no clipping) if needed
    Print half the items on cover, rest on tray card
    Adjust image up for A4 paper (instead of Letter)
